This should be someone who we can contact in your absence - a family member, a neighbour - someone who will remain in the local area during your pets stay with us.
Please include all relevant information for each pet that will be staying. Please complete 1 form per pet.
Please note that whilst Bell Reed Kennels and Cattery will always try and make contact with the owner first, in the event that we are unable to and emergency treatment/advice is needed for your pet, Bell Reed Kennels and Cattery have the right to call upon our registered veterinary practice. The owner is responsible for any bills incurred.
For Dogs: We also require evidence that your dog has been vaccinated against Kennel Cough. This must have been completed at least 14 days prior to the use of the kennels.
To comply with the current legislation on data protection, we must tell you that personal data we hold about you, why we hold it and have your permission to retain it. We store data about clients to ensure we can contact them by mail, telephone or email for use:
In an emergency; About booking queries; About special offers.
Any data we hold will not be provided to any other person or business except as required by law.
You may request to see all personal data we hold on you (We are allowed 30 days to provide it)
We only keep your data for the reasons outlined above.
We destroy your data if we have had no contact with you in 3 years.
